Resource Navigator - West Memphis, Marion Arkansas

This position is located in the West Memphis/Marion, AR area.
The Resource Navigator works in coordination with the care coordination RNs and Social Workers to provide support to program participants, their families/caregivers, and providers.
Maintains and ensures the confidentiality of all Personal Health Information (PHI) collected.
Places outreach calls to participants/parents to:
o communicate program parameters
o remind them of preventive services scheduling
o reschedule missed appointments
o schedule assessments and educations, as directed.
Serves as a member advocate through:
o ensuring access to necessary services and support
o ensuring resolution of member concerns, complaints and grievances
o assisting participant and family/guardian to successfully master skills needed for self-management of condition(s)
Places outreach calls and on-site visits to providers to:
o communicate program parameters
o request/share medical records
o ensure awareness of and coordination of services across providers
o verify/schedule appointments
o provide administrative support needed to facilitate the provision of medically necessary services (e.g., assist in DME supplies ordering on behalf of program participant)
Participates in all required program, process and quality training programs
Conducts satisfaction surveys at case closure

Supervisory

A valid drivers' license and applicable automobile insurance.
Satisfactory criminal background check and driving history.

Education and/or
Experience:
A High School Diploma or the equivalent is required
BA in Social Work or Psychology, or a Nursing Degree is preferred
A minimum of two (2) years working with behavioral health patients
Computer Skills:
Proficiency using software programs such as Microsoft Office Suite. Ability to type care coordination documentation directly into Care Management system.
Environmental Factors/Physical Demands:
Work may be performed in client's homes as well as an office environment. Travel will be required. While performing the duties of this job, the employee is required to maintain and coordinate communication; to access, input and retrieve information from various sources; and to accurately enter data into Care Management system in a timely manner.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
