This position is located in the West/Memphis, Marion, AR area.
The Social Worker Case Manager provides coordination, education and outreach services to members in the program. The Social Worker Case Manager supports the delivery of care coordination/case management services by removing barriers to the process if they fall within the realm of social work and enlists community resources. Primary responsibilities include assessing social service needs, identifying and addressing social barriers to members achieving optimal health care status, such as lack or inaccessibility of housing, transportation, utilities, or food. Additional responsibilities include assisting providers in accessing medically necessary specialty care services and ensuring members access to appropriate preventive services.

Completes psychosocial assessments.
Identifies care needs and goals based upon:
o assessment findings
o analysis of claims data
o provider input
When acting as assigned clinical coordinator, establishes Care Plan, in conjunction with the member and key providers.
When acting as assigned clinical coordinator, convenes and facilitates meetings to ensure communication and coordination between members and key healthcare and support service providers.
Monitors clinical and functional status of participants through regular face to face contact and convenes care team meetings on an ad hoc basis to address gaps in care or barriers to successful implementation of Care Plan
Maintains and ensures the confidentiality of all Personal Health Information (PHI) collected and disseminated, in accordance with HIPAA requirements
Directs, as appropriate, the Resource Navigator with regard to:
o Scheduling appointments
o Linking participant and family/guardian to community based organizations and services
o Follow up with/support for participant and family/guardian
o Communication with/support for providers
Participates in relevant and approved continuing education offerings if needed to enhance knowledge base needed to oversee care for specialty populations
Participates in all program specific training programs

Supervisory
Responsibilities: up to 2 staff as needed
A minimum of three (3) years of clinical pediatric or family experience is preferred
A minimum of three (3) years of Case Management experience is preferred
Knowledge of chronic illnesses, co-morbidities and disease management.
Knowledge of community based resources for the Medicaid population
Knowledge of adult learning and behavior change principles preferred
Clinical assessment skills, within state licensure scope of practice requirements.
Crisis intervention skills, within state licensure scope of practice requirements.
Ability to assess patient health status, within state licensure scope of practice requirements.
o Ability to travel locally with a valid drivers' license and automobile insurance.
o Criminal background check as required by the state where business is conducted
o Negative TB test annually
Education and/or
Experience:
Master's degree in Social Work
Certificates, Licenses, Registrations:
State licensed in Social Work
Computer Skills:

Proficiency using software programs such as Microsoft Office Suite . Ability to type care coordination documentation directly into Care Management system.
Environmental Factors/Physical Demands:

Work with the members in various hospitals, provider offices, and, in certain instances of high acuity, a member's home to assess continuing needs. While performing the duties of this job, the employee is regularly required to have the ability to maintain active customer and employee communication; access, input and retrieve information from the computer system; enter alpha-numeric data into a computerized system often while listening on the telephone.May be subject to repetitive motion such as typing, data entry and vision to monitor. May be subject to bending, reaching, kneeling, stooping and lifting up to thirty (30) pounds.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
